Community DVT Excellence Program
The National Blood Clot Alliance (NBCA) Community Deep Vein Thrombosis (DVT) Excellence Program recognizes outpatient and community-based centers that expand access to high-quality DVT care at the community level.
These accredited centers serve as trusted resources for patients and referring providers by delivering timely diagnosis, evidence-based treatment, coordinated follow-up, and clear pathways for escalation when advanced care is needed.
The program was intentionally designed to address critical gaps in access while strengthening referral networks and improving continuity of care for patients with suspected or confirmed DVT.
This program complements hospital-based Centers of Excellence and is not intended to replace comprehensive, inpatient VTE programs.
What Community DVT Excellence Centers Do
NBCA Community DVT Excellence Centers are recognized for their ability to:
- Ensure timely access to vascular imaging and accurate DVT diagnosis
- Provide evidence-based outpatient DVT care, including appropriate risk stratification
- Collaborate with community providers to guide patients to the right level of care, including referral for advanced intervention or specialty evaluation when indicated
- Offer patient education and longitudinal follow-up to support safe, coordinated care beyond the initial diagnosis
Program Scope & Focus
The Community DVT Excellence Program is designed for centers that manage uncomplicated DVT in the outpatient setting.
Participating centers must demonstrate clearly defined escalation and referral pathways for patients requiring advanced intervention, hematology consultation, or pulmonary embolism evaluation.
Who the Program Is For
The Community DVT Excellence Program is designed for high-functioning outpatient vein and vascular centers, or other outpatient settings with comparable infrastructure, that demonstrate:
- Expertise in venous disease
- Commitment to standardized, evidence-based care
- Capacity to provide timely diagnosis, treatment initiation, and follow-up
By recognizing qualified community-based centers, NBCA helps create a coordinated continuum of VTE care across healthcare settings.
Accreditation & Oversight
NBCA Community DVT Excellence Centers undergo a structured accreditation process that includes:
- Submission of an interest form followed by a formal application
- Review of supporting documentation and an in-person site visit
- Ongoing program reporting, education, and quality oversight
- Re-appointment every three years to ensure continued adherence to NBCA standards
